What happens when you are unable to speak for yourself? Perhaps an accident or a medical emergency. The first responders (police, ambulance, fire department) need to know who to contact but even more important, who can give them essential information about your medical history and issues. The “In Case of Emergency” (ICE) section will help them contact these people—so make they know what your wishes are when you are unable to speak for yourself.
IN CASE OF EMERGENCY (ICE) CARDS: You will notice we start with all the cards showing red-not done (or not current).
Your Action: When each action is complete and current, you will turn the card over to green. This way you can quickly scan the sheet and see what still needs to be done. There may be some cards you will choose not to complete as it’s not the time to take this action (such as the Do Not Resuscitate or Family Medical Leave Act)
YOUR CONTACT INFORMATION: This section contains important information about you and who to contact in an emergency. We included this in your Guardian so it is a handy reminder for you and for those times you may not be able to speak for yourself--when someone else needs this information.
ICE Contact Sheet: This is the page in the Guardian in the front of the ICE cards section.
Your Action:
o Fill out all the sections on the contact sheet. Use the reverse side of the sheet for any notes you want to add.
o When you complete it, please turn the card over to green
ICE Phone. There is an option on smart phones to include emergency information. It can usually be set up in the “Settings” section of your phone. In an emergency, a first responder (Ambulance) can see this information even if your phone is locked.
Your Action:
o Open your smart phone and enter your emergency information—at least who to contact and any alerts—
o Once Complete: Turn the card over to green.
ICE Wallet. Yes, it’s still important to have a “hard paper copy” for those situations when you forgot your smart phone, the battery died, or it is broken. We’ve included a wallet card.
Your Action:
o Fill it out now–it’s quick and easy
o Turn the card over to green
